What is Best in KLAS and why does it matter?
Best in KLAS is KLAS Research's annual ranking of healthcare software and services, built from a year of feedback from thousands of provider organizations. KLAS collects most of that data through researcher-led phone interviews and keeps every response anonymous, per its published methodology. That makes a #1 ranking a different signal from a juried design award: it's usability judged by the people who actually use the tool — the test that counts for incident reporting.
What did Create Ape design for PHP's incident reporting system?
Create Ape designed the UX of PHP's incident reporting system: the reporting flows, the permissions model, the forms, and the wireframes for every screen. The decisions that mattered:
- Flows shaped by HIPAA from the start. We overhauled the path a report takes from first screen to submission within HIPAA's data-handling constraints, so compliance shaped the screens rather than being layered on after.
- Permissions scoped by role. We built a permissions hierarchy around four explicit actions — create, track, submit, and edit a form — so each user sees only the actions their role uses, and nobody wades through the rest.
- Forms stripped to the necessary actions. We wireframed each screen bare-boned, surfacing only what's needed to complete each reporting step — the case study walks through the form redesign.
- Deliberately spare screens. Every extra step is a reason to skip a report, so each layout makes the one action that advances it the obvious thing on the page.
Per our project records, accessibility sat in the same compliance-aware scope: the design followed ADA accessibility requirements alongside HIPAA. Designing systems like this is part of why we pivoted the agency to medtech.

We won't claim the design caused the rankings.
